PERSON-CENTERED BEHAVIORAL SERVICES

 Ability and Empowerment Services delivery of person-centered behavioral services  to Maryland children, youth and adult residents shall focus on the following: Person and Family Driven, Individualized & Needs-Based, Array of Services & Supports, High Quality, Community-Based, Culturally and Linguistically Responsive, Early Identification & Intervention, Integrated Care, Strengths-Based and Outcomes-Based 

Admission, Screening and Eligibility determination

The management of Ability and  Empowerment Services shall ensure that staff, contractors and vendors are  informed of this policy to establish individual admission, population to be served, eligibility determination  procedures in compliance with the Maryland Department of Health and the Joint  Commission Accreditation body. 

Ability and Empowerment Services accepts for care, treatment, or services only  those individuals whose identified care, treatment, or service needs it can meet. 

Screening and Assessment.

The management of Ability and  Empowerment Services shall ensure that staff, contractors and vendors are  informed of this policy to determine the care, treatment, or services that will best meet the needs of the  individual served initially and over time.  Policy: Ability and Empowerment Services shall perform a screening and assessment  exam to all individuals enrolled in our program, and this record shall be maintained  in the individual’s file. This assessment will be performed annually per the anniversary  date of the first assessment of each individual in our program.  

Providing Care, Treatment, or Services

 The management of Ability and  Empowerment Services shall ensure that staff, contractors and vendors are  informed of this policy to determine the manner in which care, treatment or services rendered should be  coordinated among providers, between settings, independent of whether they are  provided by Ability and Empowerment Services or by an outside source, so that  the individual’s needs are addressed in a seamless, synchronized, and timely  manner. Ability and Empowerment Services shall ensure that Out-patient mental health  services both adult and minor are provided in a manner which prioritizes the  individual’s care, making any transition as seamless as possible.  

Academic Education for Minors

 The management of Ability and Empowerment Services shall ensure that staff, contractors and vendors are  informed of this policy to determine the manner in which Ability and Empowerment Services could assist a minor maintain educational and intellectual development of all admitted in our  program, using proper therapeutic academic education. Ability and Empowerment Services shall ensure that Out-patient mental health  services provided to minors include educational and intellectual activities in order  to protect minors from losing ground academically while receiving care, treatment  or services.
